Confirm the applicant and provider

Name the legal applicant, content-control arrangement, accredited provider, authorized signatory and grant-payment recipient. Supply current supporting documents and decide who submits and reconciles the application. Prior discussions do not establish a contract for this HF program.

Quote this exact scope

Three distinct national virtual case conferences in 2027, each with a 60-minute design target; secure baseline/post polling; live questions; technical rehearsal, captions and support; and an enduring companion hosted for up to 12 months if eligible. Quote any recording conversion separately and do not assume separate credit.

Define reach and recruitment

State proposed channels, profession/practice-setting mix, invitation/registration/attendance definitions, enduring starts/completers, overlap assumptions and any written guarantee. Validate or replace the planning targets: 300 verified attendees per session, 1,200 enduring starts and 1,500 deduplicated people. Give remedy terms for any guaranteed delivery.

Separate included services

Itemize hosting, secure identity, certificate technology/retrieval, accreditation governance, recruitment, routine reports, follow-up dispatch and enhanced analysis. Show which services are bundled. Remove overlaps across budget B12–B17. The prior pregnancy offer does not supply an HF price or audience commitment.

Confirm records and outcomes

Supply the field mapping for first pre responses, first post responses, all retries, item versions, verified live attendance, completions and consent. Define a privacy-preserving cross-format learner identifier. Confirm the 60-day invitation schedule, delivered/bounced denominators, data transfer and retention policy.

Accept the release responsibilities

Approve the final assessment threshold and distinct/repeat-credit rules. Supply exact credit wording and a real certificate destination. Assign responsibility for clinical review, disclosure mitigation, device/screen-reader testing, full listening and timed pilot. Confirm named reviewers can open the private materials before sharing them.

Return evidence needed for the application

An HF-specific signed scope/quote, recruitment plan and defined metrics; accreditation/applicant evidence; relevant faculty commitments; timeline; cost exclusions; data/outcomes responsibilities. No messages or requests have been sent from this package.
